Waiver, Release of Liability, Medical Authorization & Photo/Media Release

PLEASE READ CAREFULLY. THIS IS A LEGALLY BINDING AGREEMENT THAT AFFECTS YOUR LEGAL RIGHTS.

By checking the acknowledgment box and electronically signing this registration, I acknowledge that I have carefully read this Waiver, Release of Liability, Medical Authorization, and Photo/Media Release. I understand that my electronic signature has the same legal effect as my handwritten signature.

I understand that I am allowing my minor child to participate in a football event that involves inherent risks of injury. In consideration of my child being permitted to participate in the Raw Island Invitational (the "Event"), I voluntarily agree to the following:

1. Parent/Guardian Information

I certify that I am the parent or legal guardian of the registered athlete ("Athlete") and have the legal authority to execute this Agreement on the Athlete's behalf.

2. Assumption of Risk

I understand and acknowledge that participation in football activities involves inherent risks that cannot be eliminated regardless of the care taken by event organizers. These risks include, but are not limited to:

• Slips, trips, and falls
• Collisions with other participants or equipment
• Muscle strains and sprains
• Broken bones
• Dental injuries
• Concussions and other head injuries
• Heat exhaustion, heat stroke, dehydration, and other heat-related illnesses
• Cardiac events
• Permanent disability
• Death

I knowingly and voluntarily assume all known and unknown risks associated with my child's participation in the Event, including travel to and from Event activities.

3. Health Certification

• I certify that my child is physically fit and medically able to participate in the Event.
• I have accurately disclosed all known medical conditions, allergies, medications, recent injuries, surgeries, and physical limitations during registration.
• I understand it is my responsibility to notify Event staff of any changes to my child's medical condition before participation.

4. Emergency Medical Authorization

In the event of illness or injury, I authorize One Huddle One Mission, Inc., Athletes Charitable (a division of United Charitable), Raw Island Invitational, Event staff, athletic trainers, emergency medical personnel, and their representatives to obtain emergency medical treatment for my child if I cannot be reached in a timely manner.

I understand that I am solely responsible for all medical expenses incurred.

5. Release of Liability

To the fullest extent permitted by Maryland law, I voluntarily release, waive, discharge, and covenant not to sue the following entities and individuals (collectively, the "Released Parties"):

• One Huddle One Mission, Inc.
• Athletes Charitable (a division of United Charitable)
• United Charitable
• Raw Island Invitational
• Rasheed Walker
• Host schools and event venues
• Property owners and facility operators
• Coaches, officials, athletic trainers, medical personnel
• Volunteers, employees, independent contractors, security personnel
• Sponsors, vendors, exhibitors, event partners
• Officers, directors, agents, representatives
• Affiliates, successors and assigns

from any and all claims, demands, causes of action, damages, liabilities, losses, costs, or expenses arising out of or related to my child's participation in the Event, including transportation to and from Event activities, except to the extent caused by the gross negligence or willful misconduct of a Released Party, or where otherwise prohibited by applicable law.

6. Indemnification

To the fullest extent permitted by law, I agree to defend, indemnify, and hold harmless the Released Parties from any claims, damages, losses, liabilities, or expenses arising from my child's actions or omissions during participation in the Event, except where prohibited by law.

7. Medical Insurance

I understand that neither One Huddle One Mission, Inc., Athletes Charitable, United Charitable, Raw Island Invitational, nor any Released Party provides primary medical or accident insurance for participants.

I certify that my child is covered by adequate medical insurance, or I accept full responsibility for all medical expenses resulting from participation.

8. Concussion and Injury Protocol

I understand that football carries an inherent risk of concussion and other serious injuries. If Event personnel suspect that my child has sustained a concussion or other serious injury, I understand my child will be removed from participation immediately and may not return until appropriate medical clearance has been provided, if required by Event policy or applicable law.

9. Participant Conduct

I understand that athletes are expected to conduct themselves in a respectful and sportsmanlike manner and to follow all Event rules and instructions. Raw Island Invitational reserves the right to remove any participant from the Event, without refund, for unsafe behavior, misconduct, harassment, fighting, destruction of property, or failure to follow Event rules or staff directions.

10. Weather and Event Operations

I understand that Event schedules, locations, and activities may be modified, delayed, suspended, or canceled because of weather, field conditions, facility issues, safety concerns, or other circumstances beyond the Event's control. All decisions made by Event officials regarding participant safety and Event operations are final.

11. Photo, Video & Media Release

I grant permission to One Huddle One Mission, Inc., Athletes Charitable (a division of United Charitable), United Charitable, Raw Island Invitational, Rasheed Walker, Event sponsors, media partners, and their authorized representatives to photograph, record, livestream, and otherwise capture my child's name, image, likeness, voice, and participation during the Event.

I understand these materials may be used, without compensation or additional permission, in print, digital media, social media, websites, livestreams, broadcasts, advertising, promotional campaigns, fundraising materials, educational materials, and future Event marketing throughout the world and in any media now known or later developed.

I waive any right to inspect or approve the finished materials and release the Released Parties from any claims arising from the lawful use of such materials.

12. Governing Law

This Agreement shall be governed by and interpreted under the laws of the State of Maryland, without regard to its conflict of laws principles. If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

13. Parent/Guardian Certification

By electronically signing below, I certify that:

• I am the legal parent or guardian of the Athlete.
• I have read and fully understand this Agreement.
• I understand that participation in football involves inherent risks.
• I voluntarily consent to my child's participation.
• I understand that I am releasing certain legal claims on behalf of myself and my child to the fullest extent permitted by law.
• I certify that all information provided during registration is true and accurate.
• I understand that this electronic signature is legally binding.
